We declare that the information contained in this application form is correct and hereby apply to be elected as a CIWM Affiliated Organisation Member and we undertake if elected to conform to the Charter, Bylaws and Regulations of the Institution as at present formed, or as they maybe hereafter altered, amended or enlarged. We further undertake to pay the subscription prescribed, and to promote the objects of the Institution as far as may be in our power.
